We strongly recommend you have a look at this statement prior to your course start date to prevent any disappointments.

Page 1 contains the questionnaire student divers will be given when starting a course, experience or refresher. Certain answers will direct to page 2 for further clarification. Page 3 is the page that needs to be filled in together with your physician, should you have to answer a “yes” with an asterisk. The further pages explain why each of the questions on page 1 and 2 are important and what the results could be if one would go diving with certain medical conditions.
